Review the contracting and commission payout options below and select the packet that best applies to you.

Independent Agents – If your commissions pay to you as an individual, please complete and return this packet.

Agency/Firm – If you want your commissions to pay to your business OR if you have writing agents in your firm that have commissions pay to your business, please complete and return this packet.

Solicitor/Writing Agent Only – If your commissions pay to a business that you are NOT the principal or owner, please complete and return this packet. (You must also make sure that the Principal/Business Owner complete the Agency/Firm Packet above.)
